The Pre-Participation Physical Exam Form, which should accompany the student at the time of the physical, can be downloaded from the MHSA website:   This form is also available at all schools, including the Activities Office at the Warehouse, 1470 Industrial Ave. The Billings Clinic Orthopedic and Sports Medicine Center is offering special dates for school sports physicals without an appointment in August.

Dates: August 5th, 6th, 7th.
Location: Billings Clinic Orthopedic and Sports Medicine Center, 2702 8th Ave. North
Times: 3:30 pm - 4:30 pm
Cost: $20 (payable to the school of participation). 100% of the fee will be donated back to the school athletic program.
Cash or check only.

If there is a family history of premature sudden death (age 40), a personal history of fainting, passing out, chest discomfort/shortness of breath with exercise and/or any complex medical condition with ongoing treatment, we will not be able to clear them with this sports physical.  It is suggested that these students follow-up with their family physician or pediatrician. If these dates or times are not convenient, you may have the physical done by your own physician or at any other medical center.  This examination must be certified by a licensed medical professional acting within the scope and limitations of his/her practice.   If you have any questions regarding the physicals, please contact the Billings Public Schools Activities Office, Julie Glasson (281-5073) or Joni Bertsch (281-5074).



All payments and completed forms need to be turned in before the start date of each sport.  All payments are then kept for the first two full weeks of the sport.  Parents and students have those two weeks to request a refund in writing.  After two weeks, payments are then deposited and no refunds are available.
