Pre-Registration for New and Re-Enrolling Students

Once you have submitted your Pre-Registration form – this information must be reviewed and approved by school personnel. Once approved, you will receive an email with instructions for creating your PowerSchool for Parents account, or how to add this student to your current account  in order to fill out the Enrollment forms.

If you do not receive this email, please check your spam folder. If you have trouble filling out this form, or do not have access to a computer, please contact your home school for support. School Contact Information can be found on the individual school websites.
 

Age 3-5 Early Childhood Special Education Pre-Registration:

• Rimrock Learning Center serves IDEA Part C children ages 3-5 with disabilities

KinderREADy Pre-Registration:

• KinderREADy is an early literacy program for 4 and 5 year olds. Fall 2025 pre-registration is currently closed, but please visit the website and complete the interest form to be placed on a wait list.

The documents required to complete registration and finalize enrollment include:

•  Completed online enrollment.

•  Certified birth certificate.
•  Your child’s IMMUNIZATION RECORDS from a healthcare provider or  EXEMPTION FORMS:
  •  Medical Exemption Form IZ  HES101A 
  • Affidavit of Exemption on Religious Grounds Form HES – 113 
  • Note: Exemption forms can be located at the main office of the school or under the heading Immunization Requirements and Exemption Form towards the bottom of this page.
     
• Proof of address which attaches you to your address (utility bill, rental or lease agreement, or purchase agreement).
   

Any other documentation, that may pertain to your child’s enrollment:

• Legal documentation such as guardianship, binding legal documents, restraining orders, loss of parental rights, etc.

• Medical needs forms, medication forms if required

• Educational Authorization Affidavit (Caretaker Affidavit)
