New Account Request Form: This form is to request a district account for new employees. Please read the Acceptable Use Policy and Procedure completely before signing the form. By signing the form you are agreeing to abide by the policy. Fill out the form, have a supervisor sign it and then fax, email or send the form through inter-district mail using the information listed on the bottom of the form.
