Coaches Information

The district Activities Office orders supplies for middle schools. Equipment can take up to 3-4 weeks to arrive so be sure to complete the form well before the season starts.

Equipment Request Form

Complete the Extra Duty & Activity Time Sheet for assignments such as selling tickets, running the score clock, crowd control, keeping the scorebook, etc at student extra-curricular functions. Important note: Submit the Extra Duty & Activity Time Sheet within two weeks of completing the assignment.

Certified staff will be paid for all but two assignments (two hours per assignment). The hourly rate of pay is $15.50. Do not submit this form until the required unpaid activities have been completed.

Non-certified staff's hourly rate will be calculated by Payroll.

Any time a group plans to have a fund raising activity, the coach or faculty sponsor must fill out a fundraising approval request form which must be signed by the building principal.

Complete the Fundraising Report Form at the conclusion of a fundraiser. It should be turned in along with the funds to be deposited.

The participation survey should be completed at the beginning of the season (after cuts have been made). For middle schools, the form must be signed by the principal; for high schools, the form must be signed by the Activity Coordinator. Send completed forms to Leslie Croaker in the Activities office at the warehouse.

Any time subs are needed due to coaching, head coaches must complete the Sub Request and Rooming List shared Google doc for themselves and their assistant coaches who are district employees. The district does not pay for subs for volunteer coaches. It is preferable to have the sub list completed for the whole season prior to the first practice; however, if this is not possible, it must be completed a minimum of two weeks prior to the absence.  

Rooming lists must be completed a minimum of two weeks prior to an overnight trip. While the exact participants to each event may not be confirmed, the list still needs to be completed in order to secure rooms. Changes to participant names can be made closer to the time of departure. Failure to complete the rooming list in a timely manner may result in rooms not being booked.
